Continuing Care Services at Providence Place

Providence Place offers compassionate care and services, including assisted living, healthcare and rehabilitation, and memory support. The care team is attentive to accommodate the physical and safety needs of each person. For some residents, care may be delivered in the comfort of their own home through a home care service, while other residents may need added assistance.

Westchester Harbour – Assisted Living

Westchester Harbour at Providence Place (assisted living) offers the comforts of home in bright, attractive private or semi-private rooms, complemented by spacious indoor and outdoor areas perfect for spending time with friends and family. Our nurses, certified medication technicians, and CNAs create customized care plans tailored to each resident’s needs. Our community also features a secure memory care unit, engaging activity programs, and professional dining services.

Westchester Manor – Skilled Nursing

Westchester Manor at Providence Place is a licensed, skilled nursing facility, offering comprehensive health services for both short-term recovery and long-term care. Residents are often referred from hospitals to continue their recovery from illness or injury.

With 24-hour nursing care and access to physical therapy, we provide a supportive environment that fosters independence and aligns with each resident’s goals.

Long-term care services support a variety of situations for residents and can even allow a couple to reside in the same community while enjoying different levels of living. Person-centered care is at the core of all our team interactions with the goal being to add life to our residents’ years.

Residents can choose between private and semi-private rooms, each featuring a full private bathroom and comfortable personal space. Westchester Manor features dining rooms on each floor so that residents can enjoy meals and social events together. Outdoor space is available for visits and for activities.

Our residents often relax by the fireplace, visit with friends and family, read a book, or simply enjoy the welcoming atmosphere of the community. Here, every aspect of care is designed to enhance well-being and quality of life.

Rehab to Home – Skilled Nursing 

Our Rehab to Home program at Westchester Manor focuses on practical skills – gaining strength and endurance – so that short-stay residents can achieve a successful return to healthy and independent living in the community.

Under the supervision of our medical director, the nurses and therapy team customize care, which translates to a quicker recovery. Resident interests are discussed at admission, along with expectations for the rehab stay, and are used to create realistic goals.

Memory Support

Secured memory support is available at Westchester Manor (our skilled nursing facility) and at Westchester Harbour (our assisted living community) for residents who have Alzheimer’s or related dementias and need round-the-clock care. In addition to daily care, our compassionate and experienced staff help residents participate in a variety of activities designed to engage them emotionally, mentally, and physically.

Frequently Asked Questions - Healthcare Services

What can I expect on the day of admission?

Typically, paperwork is signed on the day of admission (although this can be done prior). Please bring with you the insurance card and other pertinent items, such as Power of Attorney documents and Advance Directives paperwork.

When someone transfers from the hospital to a skilled nursing facility, we often find that new residents are tired from the transition. Frequently, this is the first time since an injury or illness began that the person can sleep undisturbed. At this time, the team is also learning about the new resident, and the resident is adjusting to a new setting.

As a family member, what are my next steps after an admission?

As family, you may be invited to participate with the new resident in signing admission paperwork. (This can help facilitate the process.) On the day of admission a lot of information is shared and it can be a challenge to retain all that is discussed, so it’s helpful to be present and be an advocate.

We ask that family members and friends visit often and encourage the resident to participate in therapy as ordered and in nursing services. We also encourage self-care for family and friends – getting enough sleep, hydrating, and eating nutritious meals. In the moment, it’s hard to recognize the stress a family experiences while someone is in the hospital. Once the new resident is settled into their room, we invite you to work with the care team. Let us know about your loved one’s interests and their dislikes. This will help us get to know the new resident!

When will I start to receive therapy?

New admissions are evaluated by the therapy team, typically within 24 to 48 hours of admission. The team will work with the new resident to understand their expectations of therapy – namely, what skills are needed to transition back to the community safely, along with any activities the person enjoys. On occasion, therapy may incorporate a favorite activity as part of the person’s rehab. After all, everyone does better when a task involves something we enjoy!

How is care paid for in a skilled nursing facility?

Westchester Manor accepts Medicare, Medicaid, Medicare Advantage, commercial insurance plans, and private pay. Reach out to the Admissions Office to discuss a specific payer since many different plans have different details. A new admission must have a skillable need for insurance to pay for a stay – this could include a therapy or nursing need. Pre-admission, the hospital or your doctor will send clinical information for the facility to review and make an assessment about whether insurance will pay for the stay. For Medicare Advantage or commercial plans, pre-certification for admission is needed and is done in collaboration with the hospital and the plan.

How do I pre-register for a Rehab to Home stay?

When a surgery is scheduled, reach out to the Admissions Office at Westchester Manor and share your scheduled surgery date, hospital, and physician doing the surgery. We’ll also request a copy of your insurance cards so that we can verify benefits prior to admission. Let the nurse know that you are pre-registered for rehab at Westchester Manor; then sit back and relax! The Admissions Office will coordinate a transfer directly with the hospital.

Pre-Registering for Rehab Services

Westchester Manor makes it simple for anyone to pre-register for short-term rehab or respite services. Simply complete this form and a team member from the Admissions Office will follow up with the next steps.

Pre-registering for services allows a patient to choose a short-term rehab provider prior to a surgery or in-patient acute admission. Patients are able to focus on recovery, knowing the transition to short-term rehab has already been determined.
